Assistance with Reaching Meaningful Use and Medical Home status
Is your office workinging towards Meaningful Use or Medical Home status? If so, we are happy to assist you with achieving your tobacco cessation goals and helping you reach these milestones. In addition, we have a free electronic referral service that can easily be implemented into your electronic medical record system to refer patients who smoke to the NYS Smokers' Quitline and local cessation programs. All our services are funded by a grant from the NYS Department of Health and available to healthcare provider offices in the counties of Albany, Schenectady, Rensselaer, Greene, Columbia, Otsego, Delaware and Schoharie.
